Abstract Upravljanje sustavima kvalitete je funkcija u menadžmentu u kojoj se rad prati i mjeri te poduzimaju korektivne radnje s ciljem zadovoljavanja predviđenih ciljeva radne organizacije. To je području sestrinstva od iznimne važnosti za ishode ukupne skrbi za bolesnika što ujedno znači osiguranje kvalitete života pojedinaca i stvaranje značajnih ušteda u sustavu zdravstva. Ishodi skrbi odnose se na promjene u zdravstvenome stanju koji se mogu povezati sa svime što se učinilo tijekom procesa skrbi. To može biti stanje izliječenosti, zaliječenosti, smrti ili nesposobnosti. U to se može uključiti i osjećaj zadovoljstva ili nezadovoljstava bolesnika (udobnost, čistoća, odnos osoblja prema njima...). Pod pojmom upravljanja kvalitetom podrazumijeva se višedimenzionalni pristup upravljanju aktivnostima radi ostvarenja ciljeva radne organizacije, što uključuje najvišu razinu izvrsnosti procesa i usluge te stvaranje prednosti pred drugim zdravstvenim radnim organizacijama koje se bave istim poslovima. Sustav upravljanja kvalitetom tijekom vremena je postajao sve složeniji jer su se mijenjali zahtjevi korisnika usluga. Napredak medicine, tehnike i tehnologije značajno je doprinio promjenama u procesu rada, te se postupno razvijala potreba za potpunim upravljanjem kvalitete (engl. TQM - Total Quality Management). Da bi to stanje duha radne organizacije uspješno primijenilo u praksu, radna organizacija mora ulagati u stručno usavršavanje svojih djelatnika, treba jačati timski rad i osiguravati dobru međusobnu komunikaciju, odnosno u ukupnu organizacijsku kulturu. Dakle, potpuno upravljanje kvalitetom može se postići tek harmonizacijom svih procesa, procedura i međuljudskih odnosa unutar radne organizacije. Jedan od važnih dijelova upravljanja kvalitetom je primjena ISO standarda. ISO standardima se standardizirala zahtijevana kvaliteta usluge i značajno su utjecali i na kvalitetu usluga u zdravstvu. Danas se u svijetu najčešće primjenjuju tri metode za poboljšanje kvalitete: MBO (engl. Management by Objective/ Upravljanje s pomoću ciljeva), Demingov procesni krug kvalitete (engl. PDCA / P – Plan (planiraj), D – Do (učini), C – Check (provjeri) i A – Act (djeluj) (Sl) te Šest Sigma upravljanja kvalitetom. Medicinske sestre, s obzirom na njihov broj i vrstu posla koji obavljaju, ključ su za provođenje kvalitetne skrbi te upravljanje troškovima unutar zdravstvenih ustanova. Poznato je da one jako dobro poznaju prednosti i slabosti bolničkih sustava i da u najvećem broju sudjeluju u izravnom procesu skrbi, pa je njihova uloga u upravljanju kvalitetom nemjerljiva.
Abstract (english) Management of quality systems is a management function in which work is monitored and measured and corrective actions are taken with the aim of meeting the intended goals of the work organization. In the field of nursing, this is extremely important for the outcomes of overall patient care, which also means ensuring the quality of life of individuals and creating significant savings in the healthcare system. Outcomes of care refer to changes in health status that can be related to everything that was done during the care process. It can be a state of healing, recovery, death or incapacity. This can also include the patient's sense of satisfaction or dissatisfaction (comfort, cleanliness, attitude of the staff towards them...). The term quality management refers to a multidimensional approach to the management of activities to achieve the goals of the work organization, which includes the highest level of process and service excellence and the creation of advantages over other health work organizations engaged in the same work. The quality management system has become more and more complex over time as the requirements of service users have changed. The progress of medicine, technique and technology significantly contributed to changes in the work process, and the need for total quality management (TQM - Total Quality Management) gradually developed. In order to successfully apply this state of mind of the work organization in practice, the work organization must invest in the professional training of its employees, strengthen teamwork and ensure good mutual communication, that is, in the overall organizational culture. Therefore, complete quality management can only be achieved by harmonizing all processes, procedures and interpersonal relationships within the work organization. One of the important parts of quality management is the application of ISO standards. ISO standards standardized the required quality of service and had a significant impact on the quality of services in healthcare. Today, three methods for improving quality are most often used in the world: MBO (Management by Objective), Deming's quality process circle (PDCA / P - Plan (plan), D - Do (do), C - Check and A - Act (Sl) and Six Sigma of quality management Nurses, given their number and the type of work they perform, are the key to quality care and cost management within healthcare institutions that they know very well the strengths and weaknesses of hospital systems and that the largest number participate in the direct process of care, so their role in quality management is immeasurable.
